Nutrition support as part of wider care.
I accept enquiries concerning adults, teenagers and children when nutrition support falls within my scope. Consultations are online and provided in Polish.
I can support practical implementation of nutrition recommendations, regularity, weight goals, meal tolerance, food organisation and difficulties in the relationship with food. I do not replace diagnosis, medical treatment, psychological care or psychiatric care.
Minimal information at first, documents only through an agreed channel.
Short enquiry
The doctor, patient or guardian selects the referral option and provides contact details and a general goal only.
Scope check
I confirm whether the enquiry falls within my scope and whether different or parallel care is needed.
Consent and information
The method for sharing documents and communicating with the referrer is agreed individually and with the client's consent.
Consultation and next steps
After the consultation, we agree the plan and, where agreed, the scope of feedback to the referrer.
The website form is not for clinical documents.

- For a minor, contact is initiated by a parent or legal guardian.
